I know that background hurts your eyes, just the one where everything is moving is nearly as likely to drive you insane as well.

There are better ones, and in terms of some of them, I'd probably say:

Brutal Mario (because of the boss battles being exceptionally good)

Super Demo World and sequels (which is basically massively hacked Mario World)

Super Mario Infinity and sequels (probably the most complex storyline ever in a ROM hack for one thing...)
